By submitting this form, you (“Athlete” or “You”) agree to the following terms and conditions with Fan Day Nation (“Company,” “We,” or “Us”):

1. Use of Information & Media. You grant Fan Day Nation the right to collect, store, and use the personal information, photos, videos, and game footage you provide for the purpose of creating, publishing, and promoting content related to your Athlete Promotion Package.

2. Promotional Rights. You authorize Fan Day Nation to share your provided media and information on digital platforms, including but not limited to social media, websites, and marketing materials, to help increase your visibility and reach.

3. Accuracy & Ownership. You confirm that all submitted information and media belong to you or that you have permission to share them, and that all details provided are accurate to the best of your knowledge.

4. Privacy & Security. Fan Day Nation will handle your personal data responsibly and will not sell or share it with unaffiliated third parties, except as needed to deliver the services included in your promotion package.

5. Revisions & Updates. Fan Day Nation reserves the right to update these terms as needed. Continued use of our services after changes implies your acceptance of the revised terms.
